Customer Survey Terms and Conditions

Eligibility: This survey is open to legal residents of the United States, aged 18 years or older. Excludes employees, agents, directors, and immediate family members of Optimeal, its parent companies, affiliates, and subsidiaries.

Survey Period: The survey begins on January 8, 2024, and ends on January 26, 2024. Entries submitted after the closing date will not be considered.

How to Enter: Participants must complete the Optimeal survey. One entry per person. No purchase is necessary to enter or win. A purchase does not increase the chances of winning.

Prizes: Three participants will each win a prize of $100. The prize will be awarded in the form of Amazon gift card. Limit one prize per person.

Winner Selection and Notification: Winners will be selected in a random drawing from all eligible entries received during the Survey Period. Winners will be notified via email within ten business days after the drawing. If a winner cannot be contacted, is ineligible, or fails to claim the prize within ten business days the prize may be forfeited, and an alternate winner selected.

General Conditions: By participating, entrants agree to be fully unconditionally bound by these Terms and Conditions, and represent and warrant that they meet the eligibility requirements.

Data Protection and Privacy: By completing the survey, participants agree that Optimeal may store and use the participants' personal data for the purposes of administering the survey. All personal information will be used in accordance with Optimeal’s Privacy Policy.

Amendments: Optimeal reserves the right to cancel, terminate, modify or suspend the survey for any reason.
